Appointment Changes & Cancellation Policy

We understand that life happens, and sometimes plans change. If you need to cancel or reschedule your appointment, please keep the following in mind:

Regular Appointments

Please provide at least 48 hours' notice if you need to cancel or reschedule. A $35 fee will apply for late cancellations or missed appointments.

Procedures

For procedures, we kindly ask that you notify us at least one week in advance if you need to cancel or change your appointment.

Emergencies

We recognize that true emergencies can't be planned. If an urgent situation arises, please let us know as soon as possible and we will take that into consideration.

New Patient Instructions

To help us provide the best possible care at your first visit, please remember:

Arrive 15 Minutes Early

If you have not completed your forms in advance, please plan to arrive 15 minutes before your scheduled appointment time.

Bring Insurance Card & Photo ID

Please have both your current insurance card and a valid photo ID ready at check-in.

List of Current Medications

Provide a complete list of all medications you are currently taking, including dosages and any supplements.

Bring Relevant Medical Records

Bring any records from recent hospitalizations or ER visits, if applicable, to give Dr. Maher a complete picture of your health.
